Educating young people about how to interact with patients at the end of their lives is challenging. A qualitative study based on Husserl’s phenomenological approach was performed to describe the learning experience of secondary education students after watching, analysing, and reflecting on two videoclips featuring Cameron Duncan, a young man suffering from terminal cancer. Students from three vocational centres providing training in ancillary nursing, pharmacy, and dependent care in the Community of Madrid visited the Palliative Care Hospital. A total of (...) 110 students, with a median age of nineteen years, participated in the study. The students’ learning experience through the videoclips reveals the essential aspects required in palliative care, since the character suffers in every dimension of his being: physically as well as psychologically, socially, and spiritually. Therefore, this requires healthcare professionals to understand their experience, putting themselves in their place, to provide affectionate care and to display communication skills leading to a quality helping relationship. Four comprehensive educational categories were identified in the study. The students learned the importance of: 1) providing comprehensive and affectionate care to patients, 2) the need for communication skills in caring for patients, 3) being aware of the end of life and time left, 4) valuing life and fighting for what one wishes to attain. A visit to a PC hospital and the viewing of videoclips and reflection upon these, represents a useful strategy for secondary education healthcare students. The study indicates that videoclips are an innovative method for becoming aware of the various issues pertaining to palliative care. (shrink)

Traveling-wave solutions of the standard and compound form of Korteweg–de Vries–Burgers equations are found using factorizations of the corresponding reduced ordinary differential equations. The procedure leads to solutions of Bernoulli equations of non-linearity 3/2 and 2 (Riccati), respectively. Introducing the initial conditions through an imaginary phase in the traveling coordinate, we obtain all the solutions previously reported, some of them being corrected here, and showing, at the same time, the presence of interesting details of these solitary waves that have been (...) overlooked before this investigation. Given the restrictions of space (a limitation they too had to contend with), we can only respond to a few aspects of their interesting (...) remarks. We shall reply of course on the basis of our own approach—an approach that sets out to integrate culture, behavior, and experience, and that might be termed contextual phenomenology, phenomenological behaviorism, or perhaps most accurately (albeit more awkwardly), the phenomenological–behavioral perspective.In her commentary, Ibáñez-Guerra (2008), referring to the target articles “Phenomenology and behaviorism” (Pérez-Álvarez, Sass, and Cangas 2008) and “The role of superstition in psychopathology” (García-Montes et al. 2008), wonders whether our maintaining of the spirit of Skinner, as well as our recourse to phenomenology, might not themselves represent forms of superstitious behavior. She points out that it is not always a good idea to put new wine in old barrels. For her, the new wine and new barrels would be represented by social constructivism, of which she is reminded by some aspects of our work. But in making her own case, Ibáñez-Guerra (2008) reveals certain common prejudices concerning behaviorism, prejudices that have apparently persisted even after reading our article. Thus, for example, she does not see operant behavior as incorporating intentionality (what, in the target article, is presented in terms of final causality), because she assumes that intentionality would necessarily imply a mentalist and dualist notion involving a supposed internal mental entity that causes behavior. Skinner (1974) himself noted and rejected this very assumption in a passage that clearly distinguishes his own approach from what he calls a “stimulus-response formula”: “Possibly no charge,” he wrote,is more often leveled against behaviorism or a science of behavior than that it cannot deal with purpose or intention. A stimulus-response formula has no answer, but operant behavior is the very field of purpose and [End Page 281] intention. By its nature it is directed toward the future: a person acts in order that something will happen, and the order is temporal.(pp. 55–6)The place Skinner gives to intentionality and purpose is elaborated in our target article.Furthermore, Ibáñez-Guerra (2008) assumes that, on methodological grounds, Skinner rejects all that is not observable, or at least that cannot be operationalized. This, however, is simply incorrect, at least as regards Skinner’s radical behaviorism. As the reader may recall, radical (as opposed to methodological) behaviorism is neither operationalist nor does it reject private or subjective events because of their unobservability. Indeed, Skinner specifically acknowledges that such events are in fact observable—at least for one person: the subject him- or herself. The question for Skinner is to understand how the part of the world that is only observable and experiential for oneself is constructed or formed. In the end, it turns out that radical behaviorism is more radical, in the Aristotelian ontological sense (see Pérez-Alvarez, Sass, and García-Montes 2008), than post-modern constructionism. Indeed, as we remarked, radical behaviorism can still surprise us today. Similarly, phenomenology is starting to be missed in the clinical context (Andreasen 2007). We believe that radical behaviorism and phenomenology, mutually readjusted, are in fact excellent wines for the times we live in—if only we knew how to appreciate them.In their commentary on “Phenomenology and behaviorism: A mutual readjustment,” Fletcher and Hayes (2008) argue that the philosophy and theory of Acceptance and Commitment Therapy (ACT)/Relational Frame Theory (RFT) already contain the salutary aspects of phenomenology that we highlight in our target article. Their insistence on reducing the affinities we find between phenomenology and behaviorism to ACT/RFT consists, in our view, in a scientific–technical argument that is more scientistic than strictly philosophical in nature. Tsirelson’s problem deals with how to model separate measurements in quantum mechanics. In addition to its theoretical importance, the resolution of Tsirelson’s problem could have great consequences for device independent quantum key distribution and certified randomness. Unfortunately, understanding present literature on the subject requires a heavy mathematical background. In this paper, we introduce quansality, a new theoretical concept that allows to reinterpret Tsirelson’s problem from a foundational point of view. The essay presents an interpretation of Hegel’s speculative sentence that, emphasizing the “counter-thrust” that the modern subject undergoes here, allows us to connect the movement of the Hegelian subject to the negativity recognised by Kant at the bottom of the faculty of judgment with regard to its aesthetic dimension. This way, it aims, first, to put into question the interpretation that sees in Hegel’s philosophy a regression with regard to the consciousness, attained by Kant, of the finitude, or the constitutive (...) split, of the modern subject, and, finally, in line with recent interpretations of Hegel, like those of Žižek or Malabou, to contribute to the questioning of the traditional interpretation of Hegel’s dialectics as the overcoming of negativity into a final reconciling synthesis. Sacrificial dilemmas such as the trolley problem play an important role in experimental philosophy (x-phi). But it is increasingly argued that, since we are not likely to encounter runaway trolleys in our daily life, the usefulness of such thought experiments for understanding moral judgments in more ecologically valid contexts may be limited. However, similar sacrificial dilemmas are experienced in real life by animal research decision makers. As part of their job, they must make decisions about the suffering, and often the (...) death, of many non-human animals. For this reason, a context-specific investigation of so-called “3R dilemmas” (i.e., dilemmas where there is a conflict between the principles of replacement, reduction, and refinement of the use of animals in research) is essential to improve the situation of both non-human animals and human stakeholders. An approach well suited for such investigation is experimental philosophical bioethics (“bioxphi”), which draws on methods similar to x-phi to probe more realistic, practical scenarios with an eye to informing normative debates and ethical policy. In this article, we argue for a need to investigate 3R dilemmas among professional decision-makers using the tools of bioxphi. In a first step, we define 3R dilemmas and discuss previous investigations of professionals’ attitudes in such cases. In a second step, we show how bioxphi is a promising method to investigate the whys and hows of professional decision-making in 3R dilemmas. In a last step, we provide a bioxphi template for 3R dilemmas, give recommendations on its use, explore the normative relevance of data collected by such means, and discuss important limitations. It is unknown whether hospital-based medical professionals in Spain educate patients about advance directives. The objective of this research was to determine the frequency of hospital-based physicians’ and nurses’ engagement in AD discussions in the hospital and which patient populations merit such efforts. A short question-and-answer-based survey of physicians and nurses taking care of inpatients was conducted at a university hospital in Madrid, Spain. In total, 283 surveys were collected from medical professionals, of whom 71 per cent were female, with (...) an average age of thirty-four years. Eighty-four per cent had never educated patients about ADs because of lack of perceived responsibility, time, or general knowledge of ADs. Patient populations that warranted AD discussions included those with terminal illnesses, chronic diseases, and elderly patients. Regarding degree of AD understanding in medical professionals: 57 per cent of medical professionals claimed sufficient general knowledge of ADs, 19 per cent understood particulars regarding AD document creation, and 16 per cent were aware of AD regulatory policies. Engagement in AD discussions was considered important by 83 per cent of medical professionals, with 79 per cent interested in participating in such discussions themselves. The majority of hospital physicians and nurses do not educate their patients about ADs, despite acknowledging their importance. Patient populations of highest priority included those with terminal diseases or chronic illness or who are of advanced age. (shrink)

Background: By providing healthcare to adolescents, a major opportunity is created to help them cope with the challenges in their lives, develop healthy behaviour and become responsible healthcare consumers. Confidentiality is a major issue in adolescent healthcare, and its perceived absence may be the main barrier to an adolescent seeking medical care. Little is known, however, about confidentiality for adolescents in primary care practices in Spain.Objective: To ascertain the attitudes of Spanish family doctors towards the right of adolescents to confidentiality (...) in different healthcare situations and in the prescription of treatment.Method: A descriptive postal questionnaire was self-administered by family doctors.Results: Parents of patients under 18 years are always informed by 18.5% of family doctors, whereas parents of those under 16 years are informed by 38.8% of doctors. The patients are warned of this likelihood by 79.3% of doctors. The proportion of doctors supporting confidentiality for adolescents increases with the age and maturity of the patients, whereas workload and previous training has a negative effect.Conclusions: Spanish laws on adolescent healthcare are not reflected by the paternalistic attitude that Spanish primary care doctors have towards their adolescent patients. Doctors need to be provided with up-to-date and clinically relevant explanations on contemporary legal positions. In primary care, more attention should be paid to adolescents’ rights to information, privacy and confidentiality. Doctors should be more aware of the need to encourage communication between teenagers and their parents, while also safeguarding their patients’ rights to confidential care. (shrink)

The construct of the self is important in the domain of memory research. Recent work has shown that person memory is influenced by similarity of social targets to the self. The current experiments investigate self-similarity as defined by traits and political ideology to better understand how memory for social targets is organised. Across three experiments, participants formed positive or negative impressions based on each target’s picture, a trait-implying behavior (Experiments 1 & 2), and/or political ideology (conservative/liberal label in Experiment 2; (...) political-ideological belief statements in Experiment 3) followed by a memory test. Results showed a self-similarity effect dependent on valence in Experiment 1, but not in Experiments 2 or 3 when participants processed ideological information associated with targets. These results suggest that self-similarity has an effect on memory for social targets, but that ideological information disrupts self-focused processing of others, suggesting that ideological information also has a powerful influence on what people remember about others (i.e. social targets). This paper offers an alternative perspective to the traditional interpretation of Hegel's philosophical reflection on history, departing from a reinterpretation of Hegel's reading of the tragic action of Antigone in Chapter VI of the Phenomenology of Spirit. The customary interpretation of this text affirms that Hegel shows how the conflict of tragic action finds its truth and its end in the identity of spirit. Tragic conflict is left behind to the same extent that spirit sublates the Greek ethical substance. This (...) way, spirit can guarantee that our historical time is released from the past of the substance, or the spiritual movement of mediation from the immediacy of an ‘in-itself’. My reading, by contrast, finds under the tragic conflict of this text of Hegel's nothing but the ‘no’ of death that negates itself, or a principle that has the form of an original and irreducible conflict. Under this interpretation of Hegelian spirit, it becomes clear that it can neither fail to posit some form of ‘in-itself’ nor sublate its own tragic nature. This way it is shown that Hegel's reflection on the past does not reassure the superiority of the identity of the present, but rather illuminates its ‘broken’ nature. I thus offer an alternative view on Hegel's comprehension of the relation between present and past and between philosophy and time. (shrink)

Este trabajo parte de la constatación del carácter central que en la reconocimiento adorniano de lo estético tiene la rehabilitación de su vínculo al conocimiento y a la verdad. Se reconstruye así la crítica de Adorno a la teoría estética kantiana desde su rechazo de la separación que Kant estableció entre el ámbito estético y el epistemológico. Esta crítica, que acusa a la estética kantiana de subjetivista, se retrotrae finalmente a la insatisfacción de Adorno respecto al enfoque trascendental, en tanto (...) que no dialéctico. En este punto deviene patente la necesidad de determinar la especificidad de su propia comprensión de lo estético y de la dialéctica. Sorprendentemente se descubre entonces una inesperada cercanía entre su posición y la de la misma estética kantiana que permite reconocer en ésta última ciertos rasgos que desbordan el propio sistema crítico hacia el planteamiento adorniano. Pero esta cercanía prueba también la inmanencia y legitimidad de la (ambigua) crítica de Adorno a Kant y, de resultas, el carácter insatisfactorio de la determinación kantiana del conocimiento y la verdad. Background: Sharing information with relatives of elderly patients in primary care and in hospital has to fit into the complex set of obligations, justifications and pressures concerning the provision of information, and the results of some studies point to the need for further empirical studies exploring issues of patient autonomy, privacy and informed consent in the day-to-day care of older people.Objectives: To know the frequency with which “capable” patients over 65 years of age receive information when admitted to hospital, the (...) information offered to the families concerned, the person who gives consent for medical intervention, and the degree of satisfaction with the information received and the healthcare provided.Method: A descriptive questionnaire given to 200 patients and 200 relatives during the patients’ stay in hospital.Results: Only 5% of patients confirmed that they had been asked whether information could be given to their relatives. A significantly higher proportion of relatives received information on the successive stages of the care offered than did patients themselves. As the age of the patients increased, so the number who were given information, understood the information and were asked for their consent for complementary tests decreased. The degree of satisfaction with the information offered was high for both patients and relatives , despite the irregularities observed.Conclusions: The capacity of elderly patients to participate in the decision-making process is frequently doubted simply because they have reached a certain age and it is thought that relatives should act as their representatives. In Spain, the opinion of the family and doctors appears to play a larger role in making decisions than does the concept of patient autonomy. (shrink)

Background: Medical confidentiality underpins the doctor–patient relationship and ensures privacy so that intimate information can be exchanged to improve, preserve, and protect the health of the patient. The right to information applies to the patient alone, and, only if expressly desired, can it be extended to family members. However, it must be remembered that one of the primary tenets of family medicine is precisely that patient care occurs ideally within the context of the family. There may be, then, certain occasions (...) when difficulties will arise as to the extent of the information provided to family members.Objectives: This study aimed to describe family doctors’ attitudes to confidentiality and providing patient information to relatives as well as their justifications for sharing information.Method: A descriptive postal questionnaire was self-administered by family doctors.Results: Of 227 doctors, 95.1% provided information to a patient’s family and over a third disclosed information to others without prior patient consent.Conclusions: The findings reveal that family doctors should pay more attention to their patients’ rights to information, privacy, and confidentiality, and reflect very carefully on the fine balance between this and the occasional need for the support and collaboration of family members in delivery of care. Emphasis should be placed on ethics and legal problems during undergraduate education and in-service training of doctors. (shrink)

Lower extremity non-contact soft tissue injuries are prevalent in elite futsal. The purpose of this study was to develop robust screening models based on pre-season measures obtained from questionnaires and field-based tests to prospectively predict LE-ST injuries after having applied a range of supervised Machine Learning techniques. One hundred and thirty-nine elite futsal players underwent a pre-season screening evaluation that included individual characteristics; measures related to sleep quality, athlete burnout, psychological characteristics related to sport performance and self-reported perception of chronic (...) ankle instability. A number of neuromuscular performance measures obtained through three field-based tests [isometric hip strength, dynamic postural control and lower extremity joints range of motion ] were also recorded. Injury incidence was monitored over one competitive season. There were 25 LE-ST injuries. Only those groups of measures from two of the field-based tests, as independent data sets, were able to build robust models [area under the receiver operating characteristic curve score ≥0.7] to identify elite futsal players at risk of sustaining a LE-ST injury. Unlike the measures obtained from the five questionnaires selected, the neuromuscular performance measures did build robust prediction models. The inclusion in the same data set of the measures recorded from all the questionnaires and field-based tests did not result in models with significantly higher performance scores. The model generated by the UnderBagging technique with a cost-sensitive SMO as the base classifier and using only four ROM measures reported the best prediction performance scores. The models developed might help coaches, physical trainers and medical practitioners in the decision-making process for injury prevention in futsal. (shrink)

This article captures and critiques a recurring and prominent political argument against animal welfare improvements in Switzerland which we term the “ranking argument”. This states that Swiss animal welfare law ranks among the strictest in the world, therefore no improvements are called for. This argument was advanced three times by Swiss government authorities in 2022 alone, but also in a case dating back to 1984, to advise the electorate on popular initiatives aiming at animal welfare improvements. We argue that, while (...) the argument commits a fallacy of relative privation and is ethically dubious, it can be deployed to great effect by agents opposed to norm change in animal welfare regulation. We conclude with some thoughts on how the ranking argument can and should be challenged in public discourse. (shrink)

Background: Doctor–patient sexual relationship is considered to be unfair because the first party would be abusing the second party’s vulnerability. The prohibition of this relationship is noted in the Hippocratic oath. Currently, a reprise of the use of oaths in medical schools can be observed.Aim: To determine whether the prohibition has been maintained and how its expression has varied in the oaths during different periods.Methods: 50 oaths were studied: 13 ancient–medieval and 37 modern–contemporary. Of the 50 texts, 19 were versions (...) of the original oaths. The oaths that pointed out the prohibited doctor–patient relationship referred to any sexual aspect or included paragraphs that began as the Hippocratic oath does were noted.Results: Of the 24 texts that expressed the prohibition, 8 were ancient–medieval and 16 were modern–contemporary. Some expressly call it Hippocratic oath, many use general terminology and others describe it in association with other commitments .Conclusions: The clause on the prohibition of the doctor–patient sexual relationship in Hippocratic oath was included to be for legal, economic and social reasons at the time. That the clause is found mostly in the ancient–medieval oaths can be attributed to the influence of the original. This commitment is generalised and associated with others by contemporary formulas. Currently, sexual relationships are the subject of legal and ethical analysis and their inclusion in the oaths is being debated. (shrink)

The paper explores different possibilities in order of maintaining a compatibilism between free will, in a strong sense, and determinism. The notion of determinism is analyzed in deep. It is defended a general conception of free will as a certain kind of mental causation in absence of fatalism. Also, it is argued that other compatibilist possibilities would be possible inside that general conception of free will, being some of them more radical than other ones.

Durante décadas, excepción hecha de la llamada lógica intuicionista. la peor acusación que se le pudo hacer a un lógico fue la de psicologista. Sin embargo, lógica y psicología. tal y como pretende sena larse en este trabajo, no sólo están mucho más cercanas de lo que pudie ra parecer sino que. también, viven en una relación complementaria.

A partir de la reivindicación de Adorno del poder crítico del arte, este trabajo confronta la posición de este pensador con las de Kant y Hegel a propósito de la cuestión de la autonomía del ámbito estético. Explica por qué Adorno considera que ni Kant, quien afirma esa autonomía, ni Hegel, quien, por el contrario, asume su heteronomía, logran reconocer (el poder de) la obra de arte y de la experiencia estética. Muestra luego que Adorno tomó conciencia de que ello (...) proviene del compromiso de ambos autores con la pretensión de plena autonomía del sujeto moderno, y explica finalmente que Adorno sí reconoce la función crítica del arte y del pensamiento que le corresponde gracias a que rechaza tal pretenciosa subjetividad. Taking Adorno's vindication of the critical power of art as a starting point and focussing on the question of the autonomy of the aesthetic field, this paper confronts Adorno's position with those of Kant and Hegel. I explain why Adorno considers that neither Kant-affirming its autonomy- nor Hegel -assuming its heteronomy- succeeded in recognising (the power of) the work of art and the aesthetic experience. I show that Adorno saw that this was due to their common commitment to the modern subject's claim to full autonomy, and finally argue that Adorno, on the contrary, can do that because he abandons such a pretentious subjectivity. This work begins by proposing the need for exploring the mode of being of mental disorders. It is a philosophical study in an Aristotelian perspective, with special emphasis on the anthropological–cultural dimension. It is difficult for such an inquiry to be carried out from within psychiatry or clinical psychology, committed as these fields are to their own logic and practical conditions. The issues are, in any case, more ontological than strictly clinical in nature. We therefore turn to Aristotle, and specifically (...) his doctrine of the four “causes,” to flesh out the social and cultural dimensions of mental disorders. In accordance with the present analysis, the material cause of disorders would be found in the contingencies of life; the formal cause would pertain primarily to the way clinical conditions themselves can serve as models of ‘being ill’ in our society; the efficient cause would correspond to the patients themselves, understood as active (albeit less than fully conscious) agents as well as to the pharmaceutical industry and the mass media; the final cause would be found in different adaptive functions served by the disorder. We conclude that the “mode of being” of most (if not all) mental disorders—in particular, their status as mental disorders—can often have more to do with cultural forms than with biological factors. In Spain, any person under the age of 18 is a minor. Generally, minors lack the legal capacity to take legally binding actions because they are deemed incapable of legally binding consent. Spanish civil law recognises, however, that the child, in accordance with the law and being sufficiently mature, may act for himself. It stands, then, that consent, as expressed by the “sufficiently mature” minor, should be legally valid.

We present evidence indicating new individual differences with people's intuitions about the relation of determinism to freedom and moral responsibility. We analysed participants' written explanations of why a person acted. Participants offered one of either 'decision' or 'causal' based explanations of behaviours in some paradigmatic cases. Those who gave causal explanations tended to have more incompatibilist intuitions than those who gave decision explanations. Importantly, the affective content of a scenario influenced the type of explanation given. Scenarios containing highly affective actions (...) (e.g. murder) tended to generate more decision explanations than scenarios with low affective content (e.g. cheating on taxes). These results give important clues about the proximal processes generating some intuitions about free will and moral responsibility. (shrink)

The present work attempts to evaluate in what manner the Marian virtues marked out throughout the Middle Ages with respect to women, contribute powerfully to the dignification of women and their redemption from the traditional accusations that they are a perverting influence on humanity.

This paper proposes to call «countermodern aesthetics» a philosophical line that, reaching from the last Kant to Heidegger, would oppose to the modern dominance of theory while acknowledging the inner connection of the aesthetic to truth, to the tragic truth. It presents then cinema as a mass medium that modern aesthetics interprets according to its own conception of art in order to fit it into the advanced capitalist system and its ideology. And, finally, ir argues that S.Zizek´s alternative understanding of (...) cinema, because of the tragic condition of its very underlying metaphysics, continues the tradition that has been presented as «counter-modern aesthetics».
